Mountain Laurel Medical Center is Accredited by ADCES to Provide Quality Diabetes Self-Management Education and Support
Oakland, MD — Mountain Laurel Medical Center has achieved the significant milestone of accreditation by the Association of Diabetes Care & Education Specialists (ADCES). Accreditation represents a high level of quality and service to the community, and the ability to better meet the needs of Medicare recipients and others affected by diabetes in the Western Maryland Region.

“Congratulations to Emilee Friend and her entire team that creates, improves, supports, and provides services for Mountain Laurel Medical Center’s ADCES DSME Program. Mountain Laurel Medical Center’s program is ‘Glucose to Goal.’ As a provider, I proudly and confidently refer my patients with diabetes to this program.” – Chief Medical Officer Shawn Long.

Diabetes education services utilize a collaborative process through which people with or at risk for diabetes work with a diabetes educator to receive individualized care to help them reach their health goals. Diabetes education is a covered Medicare benefit when delivered through an accredited program.

“Evidence shows diabetes education and support is helping people with diabetes to lower their A1c’s and obtain the knowledge and skills to better self-manage their condition,” said Sacha Uelmen, Director of Accreditation at ADCES.

ADCES is a National Accredited Organization (NAO) for the Centers for Medicare and Medicaid Services (CMS). Accredited programs must meet the 2017 National Standards for Diabetes Self-Management Education and Support; 10 standards that are revised every 5 years by a group of diabetes educators representing both ADCES and the American Diabetes Association and approved by CMS. Programs must maintain compliance with the Standards during the 4-year accreditation cycle.

About ADCES:
ADCES, formerly known as the American Association of Diabetes Educators (AADE), is a multi-disciplinary professional membership organization dedicated to improving diabetes care through innovative education, management, and support. With more than 14,000 professional members, including nurses, dietitians, pharmacists, exercise specialists, and others, ADCES has a vast network of practitioners working with people who have, are affected by, or are at risk for diabetes.

The Patient Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means to build a reliable primary health care system that delivers accessible, affordable and high-quality health care.
The Patient Centered Medical home provides Comprehensive and Coordinated Care. PCMH strives to meet the majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care and chronic care. This comprehensive care will be coordinated by a “team” of health care staff which will include physicians, advanced practice nurses and physician assistants, nurses, medical assistants, care managers, educators, social workers and the entire office staff working together to enhance the care and service within the office and community resulting in improved clear and open communication among patients and families, the medical home and the members of the broader care team.

PCMH provides for Superb Access to Care. The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced office hours and around the clock access by telephone to members of the health care team. The medical home practice is responsive to patient preferences regarding access.

Patient Centered Medical Home is a Systems-Based Approach to Quality and Safety. The primary care medical home is committed to quality care and quality improvement by ongoing utilization of evidence-based medicine and the use of tools and support guides to improve shared decision making with patients and families and the regular use of practice measurements and monitoring to improve patient care and outcomes while also measuring and responding to patient experiences and satisfaction.

Mountain Laurel has received the highest recognition as a Patient Centered Medical Home through the National Committee for Quality Assurance (NCQA). We are committed to providing high quality, team based, comprehensive and coordinated care for all patients.
