Mountain Laurel Medical Center is recognized as a Quality Leader by the Health Resources and Services Administration (HRSA)
Mountain Laurel Medical Center is the recipient of several HRSA Recognition Badges for 2021, more than any other Community Health Center in Maryland, and one of only two in Maryland to receive the GOLD Health Care Quality Leader badge!

Mountain Laurel Medical Center is an Outpatient (ambulatory care) Family Practice offering care to patients of all ages. We care for pediatric, adolescent, adult, and geriatric patients.
Board Chair Brenda McCartney says, “Mountain Laurel Medical Center prides itself on providing high-quality health care. The staff’s attention to detail and their willingness to go above and beyond make MLMC successful. To be one of two health centers in Maryland to achieve GOLD health care quality status is a remarkable achievement.”
The following are the Community Health Quality Recognition (CHQR) award badges received along with the associated criteria:
Improving Quality of Care: GOLD Health Center Quality Leaders achieve the best overall clinical quality measure (CQM) performance among all health centers. They are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%).
Access Enhancer: Recognize health centers that increase the total number of patients they serve and the number of patients who receive at least one comprehensive service (mental health, substance abuse, vision, dental, and/or enabling) by at least 5%.
Health Disparities Reducer: Recognize health centers that:
Meet or exceed Healthy People (HP) goals in the areas of low birth weight, hypertension, or uncontrolled diabetes for at least one racial/ethnic minority group.
Demonstrate at least a 10% improvement in the areas of low birth weight, hypertension, or uncontrolled diabetes for at least one racial/ethnic group.
Advancing Health Information Technology (HIT) for Quality: Recognizes health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.
Patient-Centered Medical Home Recognition (PCMH): Recognizes health centers with PCMH recognition in one or more delivery sites.

The Patient Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means to build a reliable primary health care system that delivers accessible, affordable and high-quality health care.
The Patient Centered Medical home provides Comprehensive and Coordinated Care. PCMH strives to meet the majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care and chronic care. This comprehensive care will be coordinated by a “team” of health care staff which will include physicians, advanced practice nurses and physician assistants, nurses, medical assistants, care managers, educators, social workers and the entire office staff working together to enhance the care and service within the office and community resulting in improved clear and open communication among patients and families, the medical home and the members of the broader care team.

PCMH provides for Superb Access to Care. The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced office hours and around the clock access by telephone to members of the health care team. The medical home practice is responsive to patient preferences regarding access.

Patient Centered Medical Home is a Systems-Based Approach to Quality and Safety. The primary care medical home is committed to quality care and quality improvement by ongoing utilization of evidence-based medicine and the use of tools and support guides to improve shared decision making with patients and families and the regular use of practice measurements and monitoring to improve patient care and outcomes while also measuring and responding to patient experiences and satisfaction.

Mountain Laurel has received the highest recognition as a Patient Centered Medical Home through the National Committee for Quality Assurance (NCQA). We are committed to providing high quality, team based, comprehensive and coordinated care for all patients.
