Child's Birth Certificate
Guardianship Documents (if applicable)
Parent/Guardian’s Photo ID
Child's Medical Insurance Card
Child's Physical Exam Form
Child's Shot Records
TANF/ SNAP/SSI Benefits Letter (if applicable)
2025 Tax documents (if working)
Previous month’s paystub (if working)
Documentation of pension or unemployment payments
Proof of pregnancy (if you are applying for the Prenatal Program)
